不同保温措施对头颈肿瘤手术后麻醉复苏的影响

Effect of different thermal insulation measures on the recovery from general anesthesia in patients with head and neck tumor

摘要: 目的: 探讨不同保温措施对头颈肿瘤手术后麻醉复苏的影响。方法: 收集2017 年10 月—2018 年10 月间上海交通大学医学院附属第九人民医院择期全麻下行头颈肿瘤手术、资料完整的患者125例,根据术中采用的保温措施分为常温组(A 组)31例、保温毯组(B 组)36例、温液仪组(C 组)32例和保温毯+温液仪组(D 组)26例。比较4组患者复苏期血压、体温、心率、意识恢复时间、拔管时间及并发症发生率。采用GraphPad Prism 5软件包进行数据分析。结果: 全麻复苏期患者体温、心率和血压波动B、C和D组均显著低于A 组,且D 组波动显著低于其他组(P<0.05)。意识恢复时间和拔管时间B、C和D组患者均显著低于A组,且D组显著低于其他组 (P<0.05)。复苏期并发症发生率B、C和D组患者均显著低于A 组,且D 组显著低于其他组(P<0.05)。结论: 全身麻醉下行头颈肿瘤手术患者采用保温措施,特别是综合保温措施,可明显提高复苏质量,降低并发症发生率,提高复苏安全性。

Abstract: PURPOSE: To study the effect of different thermal insulation measures on the recovery from general anesthesia in patients with head and neck tumor. METHODS: One hundred and twenty-five patients with head and neck tumor under general anesthesia from October 2017 to October 2018 were selected. According to the thermal insulation measures used during operation, the patients were divided into four groups: normal temperature group (group A, 31 patients), thermal insulation blanket group(group B, 36 patients), liquid heating meter group (group C, 32 patients) and integrated group (group D, 26 patients). Changes in blood pressure, body temperature, heart rate,extubation time, conscious recovery time and incidence of complication were compared among the four groups. The data were analyzed using GraphPad Prism 5 software package. RESULTS: During the recovery phase, changes in body temperature, heart rate and blood pressure were significantly lower in group B, C and D than in group A, and lowest in group D (P﹤0.05). Extubation time and recovery time was significantly lower in group B, C and D than in group A, and lowest in group D (P﹤0.05). The incidence of complications was significantly lower in group B, C and D than in group A, and lowest in group D (P﹤0.05). CONCLUSIONS: Thermal insulation measures, especially integrated thermal insulation, can significantly improve the quality of anesthesia recovery, reduce the incidence of complications and improve the safety of anesthesia recovery.
