口腔癌患者术后舌神经功能障碍与生活质量的关系评价

Relationship between postoperative lingual nerve dysfunction and quality of life in oral cancer patients

摘要: 目的: 通过自制问卷,调查口腔癌根治术后出现舌神经损伤患者的口腔功能及生活质量情况,为临床上管理舌神经损伤患者提供依据。方法: 通过对350例患者进行定性感觉分析,筛选出其中存在舌感觉麻木症状的40例患者,采用自制《舌感觉功能障碍康复问卷》进行问卷调查。结果: 调查结果显示,舌感觉功能障碍5种最常见表现依次为①对于感觉的改变无法恢复的恐惧感;②难以做出需要舌参与的表情;③讲话时遇到困难;④无法意识到口底或舌头上的食物残留以维持口腔卫生;⑤因舌感觉的改变而难以入眠。结论: 舌感觉功能障碍从多方面导致生活质量下降,口腔颌面外科医护人员应加强对舌神经损伤的围术期评估和管理。

Abstract: PURPOSE: To investigate the oral function and quality of life of patients presenting with lingual nerve injury after radical surgery for oral cancer by means of a self-administered questionnaire, to provide a basis for the clinical management of patients with lingual nerve injury. METHODS: Through qualitative sensory analysis of 350 patients, forty patients with tongue sensory numbness were screened out, and the questionnaire was conducted by self-made Rehabilitation Questionnaire of Tongue Sensory Dysfunction. RESULTS: According to the results, the 5 most common manifestations of tongue sensory dysfunction were ①fear of not recovering from altered sensation; ② difficulty in making expressions that require tongue involvement; ③difficulty in speaking; ④inability to be aware of food residue on the floor of the mouth or tongue to maintain oral hygiene; ⑤difficulty in sleeping due to altered tongue sensation. CONCLUSIONS: The decline in quality of life due to tongue sensory dysfunction is multifaceted and oral and maxillofacial surgery providers should enhance the perioperative assessment and management of lingual nerve injury.
